Be Prepared for Your First Visit

Welcome New Patients

After you make your first appointment, you’ll receive an email from us asking you to add your patient information to our confidential, secure and HIPAA compliant Patient Portal.

The email we send includes your security code so you can register with a username, password, full name, date of birth and zip code. Once created, you’ll be asked to answer 3 security questions to help protect your account.

Then you’ll be able to login and add your patient, insurance and health information to the online forms found in the myInfo and myDocuments dropdown menus of the portal. Other forms you may need are available online for printing and completing.

Please arrive at least 20 minutes before your scheduled appointment. We do our best to keep our schedule timely and know your time is valuable.  Please note that if you arrive more than 10 minutes late, we may have to reschedule your appointment.

What Should I Bring to My First Visit?

Please bring your insurance card(s) and photo ID.  We may have to reschedule your appointment if this information is not with you.

Please also be prepared to make your co-payment or pay deductible at your visit.  Many medical services are covered by insurance, but cosmetic services are elective and normally are not covered.  For your medical co-pay or deductible, or payment for your cosmetic appointment, we accept cash, check, or credit card.

During your visit, your provider and support staff will be learning about you and your skin care needs. To help them gather as much information as possible, you may be asked to contact your referring doctor’s office to have your relevant medical records forwarded to our office in time for your visit. These records may include:

  • Surgical reports
  • Pathology reports and, if possible, the corresponding slides, scans (CT, MRI) and X-rays
  • Laboratory test results
  • Referring and consulting doctor’s office and hospital notes

Other information that will be helpful to you and your provider include:

  • List of medications you’re currently taking (including over-the-counter remedies, vitamins and herbal supplements)
  • List of question or concerns you may have about your illness or treatment to discuss with your provider
